2% Distribution Grant Application Information

Please note that the following forms are all fillable forms that can be downloaded to your computer or completed from the website. Once the forms are filled in, print and submit them to the appropriate address as listed on the form or in the instruction packet.

 

2% Grant Application and Information
2% Grant Application

Grant applications are accepted twice each calendar year - March 31 and September 30. The application includes all instructions and must be filled out completely to be eligible for funding.

Reporting Requirements and Forms
2% Grant Application Financial Report Financial Reports are due quarterly - April 15, July 15, October 15, and January 15. 
These reports are required and must be submitted each quarter throughout the life of the grant.
2% Grant Application Progress Report Progress Reports are due semi-annually - July 15 and January 15.
These reports are required and must be submitted semi-annually throughout the life of the grant. This form is also used for the final report when your grant is complete. Although this report has always been required, this form is new and now provides for easy completion of the required reports.
